Provider Scheduling Information

Using the Online Order Form:

  • Click HERE to open the online order entry screen in a new window
  • Fill out all information the has an asterisk*
  • After the necessary information is entered,    
    click “Submit”
  • Once you click “submit,” no additional 
    editing can be done. If you realize you made 
    a mistake, you can resubmit a new order.
  • Our goal is to reach the patient as soon as 
    possible. If we cannot reach them within 
    three (3) business days, our scheduling 
    department will notify you.
  • When we reach the patient, you will receive 
    a reply with the patient’s appointment 
    information.
